We will work with the care recipient to identify their social, lifestyle, and other goals. We will then ascertain how their CHSP can support this.
Working within the parameters of the individual’s CHSP, we determine which services and support can be provided.
Using our “best fit” methodology we find the right carers and services to support the individual. We facilitate the introduction, allowing the care recipient to work directly with the carer.
Monitoring and evaluation is an important part of our process, ensuring that the care recipient is happy, supported and receiving the correct care. We check in bi-monthly for feedback.
